Lay on the floor and do triceps extensions with kettlebells. Set yourself up like you're about to bench. Lower the kettlebells until they are on the floor, pause them, then extend up. Try not to use any momentum. Squeeze the kettlebells off of the floor. On the eccentric, try to roll your elbows in and keep them high. As the video shows this may not happen but this will load the medial triceps head when you attempt to do this.
